Welcome to the Breast and Ovarian Cancer Prevention (BOCP) Clinic at Fred Hutchinson Cancer Center. Your first appointment will be at the South Lake Union Clinic. This appointment usually lasts 2 to 4 hours. We recommend that you bring a friend or family member with you to help keep track of the information that your team will be giving you.
Forms You Need for Your First Appointment
Please download, print and complete the Breast and Ovarian Cancer Prevention Intake Form. To complete and submit the form by email: click on the “download PDF” link, download and save the file to a computer. Open the file and complete, then save and close the file. Attach the saved file to an email and send to PCCBOCP@fredhutch.org. Tip: Do not fill it out in web-based format as there is a risk your work will not be saved.
If you would like to fax the form, send the completed questionnaire to 206-606-6135, Subject: [Last name] BOCP Questionnaire.
If you would like to mail the form, send the completed questionnaire to: Fred Hutchinson Cancer Center 825 Eastlake Avenue East, Attn: Wellness Center, Seattle, WA 98109.
Please send the form at least 1 to 2 weeks prior to your appointment. Your providers will review it, along with your relevant medical and family records, several days prior to meeting with you.
At your appointment, you will meet with a team of breast and ovarian cancer experts, including a medical oncologist, a genetic counselor, a nutritionist, and a gynecological oncologist. You'll receive information about hereditary and non-hereditary breast and ovarian cancer risk factors and strategies for risk reduction.
Depending on your personal history, we may ask you to share your family cancer history with us to help us prepare for your appointment. If we’ve asked for this information, you’ll receive a separate email to access Progeny, a secure, web-based tool that will go through a series of questions about your family history. To sign in, please follow the instructions in the email. Please note, you will need to fill out this questionnaire at least two weeks prior to your appointment.

Preventing Colds and the Flu
To protect the safety and well-being of all Fred Hutch patients, please read the below guidelines before your first visit. Caregivers should not come to the clinic with cold and flu symptoms.

- A book or magazine to help pass the time between appointments
Directions and Parking
Parking is available in the South Lake Union Clinic garage. The cost is up to $4.00 per day with validation.
Reminders

- Don't forget to complete the forms above and bring them with you to your appointment.
- Wear comfortable shoes and clothing.
