- Demographic information (legal name, etc.)
- Insurance information
- Related doctor and facility information
Ask your specialist(s) to provide:
- Pathology report
- Clinical notes
- Chemotherapy roadmap
- Imaging
- Records regarding significant past medical history or surgeries: operative report or metal implant record
