Your first appointment at the Gastrointestinal Cancer Prevention Program at Fred Hutchinson Cancer Center with a surgical oncologist specializing in gastrointestinal cancers will be at the South Lake Union Clinic. First appointments usually last one to two hours. We recommend that you bring a friend or family member with you to help keep track of the information that your team will be giving you.

If you would like to complete and submit the form by email, click on the “download PDF” link, download and save the file to a computer. Open the file and complete, then save and close the file. Attach the saved file to an email and send to PCCGICP@fredhutch.org. Tip: Do not fill it out in web-based format as there is a risk your work will not be saved.
If you would like to fax the form, send the completed questionnaire to 206.606.6135, Subject: [Last name] GICPP Questionnaire.
If you would like to mail the form, send the completed questionnaire to: Fred Hutchinson Cancer Center 825 Eastlake Avenue East, Attn: Wellness Center, Seattle, WA 98109.
Please send the form at least 1 to 2 weeks prior to your appointment. Your providers will review it, along with your relevant medical and family records, several days prior to meeting with you.
At the time you schedule your clinic visit, you will be asked for information about your medical history and a family history will be taken. For each of your relatives (e.g. children, siblings, parents, aunts, uncles, grandparents), questions will be asked about their:
- Age and health status
- Cancer risk factors (e.g. smoking, radiation, chemical exposures, etc.)
- Colonoscopy results, any colon polyps
- Age of any cancer diagnosis and where the cancer originated
The family history sometimes requires phone call(s) to one or more relative(s). Records may be requested for some of your relatives, and staff will provide assistance with these requests.
Depending on your personal history, we may ask you to share your family cancer history with us to help us prepare for your appointment. If we’ve asked for this information, you’ll receive a separate email to access Progeny, a secure, web-based tool that will go through a series of questions about your family history. To sign in, please follow the instructions in the email. Please note, you will need to fill out this questionnaire at least two weeks prior to your appointment.

At your appointment, you will meet with a gastroenterologist with expertise in hereditary cancer and prevention for individuals at increased risk. You may also meet with a gynecological oncologist, nutritionist and/or genetic counselor, depending on your particular situation. Each provider will make recommendations to you and your healthcare provider for cancer screening and prevention. After your appointment, we'll send you a letter that summarizes our recommendations.
In general, insurance covers the appointment. Pre-authorization is done for nutrition consultation.

Preventing Colds and the Flu
To protect the safety and well-being of all Fred Hutch patients, please read the below guidelines before your first visit. Caregivers should not come to the clinic with cold and flu symptoms.
What to Bring
- Any records or imaging CDs as requested by your patient care coordinator
- Your insurance card
- A book or magazine to help pass the time between appointments
Directions and Parking
Parking is available in the South Lake Union Clinic garage. The cost is up to $4.00 per day with validation.
Reminders
- Be sure to take note of your appointment time.
- Don't forget to complete the forms above and bring them with you to your appointment.
- Wear comfortable shoes and clothing.
