Your first appointment at the Prostate Cancer Genetics Clinic will be located in Fred Hutch Sloan Clinic 2, on the fourth floor. During this appointment, you will meet with a multidisciplinary team that includes a genetic counselor and a prostate cancer medical oncologist with expertise in genetics. The appointment will last approximately one to two hours. We recommend bringing a friend or family member to help you remember the information provided by your team.

Please download, print, and complete the following forms, and bring them with you to your first appointment. We will review them and address any questions you may have when you arrive.
In addition to the above forms, you will receive an email from CancerIQ requesting details about your family history of cancer. Please complete this request before your appointment because it may inform the discussion and what kinds of tests and/or treatments might be considered.

Preventing Colds and the Flu
To protect the safety and well-being of all Fred Hutch patients, please read the below guidelines before your first visit. Caregivers should not come to the clinic with cold and flu symptoms.

- A book or magazine to help pass the time between appointments
Directions and Parking
Parking is available in the Sloan Clinic garages. The cost is up to $4.00 per day with validation.
